No one knew how to free Regina from the robot's grip, so the team leader jumped onto a forklift and sped across the factory floor toward the landing. he ran in found a maintenance worker and dragged him outside they got on the forklift the maintenance worker had to sit on the team leader's lap and they sped back through the factory towards the robot cage 23 when they got there they took this maintenance guy and basically pushed him into the cage and told him to get Regina out, but the maintenance worker didn't speak any English, he was terrified by what he was seeing, seeing Regina as she was, he really didn't know how fix the situation.
Tempers were burning and before long the maintenance worker simply became overwhelmed and ran away when emergency crews finally arrived. Several minutes later, Regina was still pinned down by this robot. She was aware that she had her eyes wide open and she was still totally silent and they were able to get out of it. She was sent to a hospital and then transported to a trauma center, but it was too late. The damage that had been done to her by being impaled by this robot was too extensive and she died in the hospital. Regina's company agent had not adequately trained her or any of her co-workers.
The agent's policy was to hire people and get them on the floor as quickly as possible to help them reach their quota, and as a result, Regina and her team members had no idea about a lockout procedure where, if you're going in In one of these robot cages, you need to make sure the power switch is locked, meaning it won't turn back on if you fix the fault that has caused your robot to stop.
