True Sciatica VS Piriformis Syndrome

and

piriformis

syndrome

? Well, they both cause the same symptoms, but there are actually two different conditions that need to be treated differently. In today's video, Dr. David Oliver, who is a chiropractor, will talk to you about that difference. True

are often confused. There are two separate conditions, but they share very similar features and symptoms. The word sciatica is actually the problem here. to the sciatic nerve which causes pain to travel down the leg, so the sciatic nerve exits the lumbar spine here in the lower back and down the butt and down the back of the leg.

True sciatica usually comes from some kind of impingement along the lombard nerve roots, the most common cause is a herniated disc, so if you have a herniated disc like here, you may see a nerve root impingement that it can affect that sciatic nerve which will cause sciatic Piriformis syndrome symptoms on the other hand is due to sciatic type symptoms and this is where this gets confusing but you have a piriformis muscle. what joins this is your sacrum it's the base of your spine it goes from your sacrum to your hip and it joins right there it's a thin muscle it's not a huge muscle but it's a muscle that goes right there and your sciatic nerve actually runs below that if your piriformis muscle is dysfunctional or too tight or in spasm it can compress against the sciatic nerve and cause you symptoms down the leg so the real difference here is where the problem is whether it's in the lower back back or buttocks in the piriformis muscle. in itself so with any kind of sciatica symptom so any pain going down the sciatic nerve can have everything from shooting pain, burning pain, numbness and tingling or even spasms occurring in the leg so you're looking for a variety of symptoms that travel down the back of the leg essentially from the hamstring to the calf, which is usually the course of sciatic type symptoms, you can also take it to the foot when looking for general radicular pain sciatic type pain or pain running down the back so if you have

sciatica again or piriformis syndrome you can have very similar symptoms as it is very confusing but you have to differentiate this and there are a couple of different ways that you can see it most people with

sciatica or lumbar herniated disc that is causing the sciatica will have more pain when they sit because when you sit you compress the di sco what causes more herniation or more progression of the problem affecting the nerve gives more symptoms whereas when you are standing you usually feel at least a little better this is not a hard and fast rule of thumb but it is a general case for most People with this, while with piriformis syndrome when you sit down you may not have as many symptoms, your symptoms may increase when you stand up and when you stand up. actually tense that muscle a little more and you could put more compression against the side of the nerve that's one way to tell the difference from the other way is to go through some tests just to see which one is the simplest for the piriformis you just need to touch it , so literally take your hand or finger and just push it around your butt and again you're looking for the hip and the sacrum so to find your hip go right to the side II here's a bone and kind of a knobby bone if you turn your leg going in and out it's called the greater trochanter if you go from there and slide your thumb in and just poke around if it feels really tender from one side to the other you may be afraid of the anterior syndrome that is contributing through the symptoms , the other way to test the piriformis besides just palpating and touching it is if you're laying on your back on your bed or lying on the floor and you're basically going to grab that knee as you bring it towards you and you're going to cross it across your body if you feel a lot of tenderness or a lot of pain in your butt compared to the other side that's more like a sign that's that's your piriformis symptom if you're looking for a self-assessment for your lower back or more of a true sciatica or a symptom coming from the lumbar spine this contraction test is really important that's where you sit in the chair and really slouch in bad posture and really slouch your head into your legs and see if that it increases your symptoms if it does it's more of an indication that it's coming from the lower back it's confusing but they are two separate conditions and it's important that we treat them separately, piriformis syndrome, if it's just the purest form of the syndrome, we can get away with doing a lot more because we can do more stretches that involve a little bit of bending, so you might see the standard figure-four stretch where you cross a pi erna and pulled it towards you you can do some simple ball work or soft tissue work on the piriformis muscle and just try to let go we usually get more response for that whereas if it is a true symptom of m or true sciatica lumbar radiculopathy coming from the lower back we want to try to decompress that so we don't really want to go into a flexion type stress because it can make it worse a couple of those exercises on sciatica could be that cat-cow mobilization smoothing of the spine or that prone upward press again we have other videos you can reference back to this you need to perform these exercises extremely slowly to make sure they are the right one for you but again it is very important to differentiate is it is coming from your back or it comes from your butt and you treat each one accordingly that's ok we hope you enjoyed this video and if you did Yes, please give us a like and subscribe to this YouTube channel for more videos like this also if you would like us to send you three exercises to correct Forward Head and Rounded Shoulders Posture you know the slouched posture many people suffer from today we would love to send you three exercises to help you fix it If you want to get them there will be a link here in the video somewhere or there will be a link below in the description just go to that page enter your email information and we will send you these videos right away.
